Caregiver Affiliation Agreement Addendum doc to the standard affiliation agreement for conduct caregiver background checks B. Negotiated Agreements Negotiated Agreements are those by the affiliate that may contain language in conflict with relevant Wisconsin State Statutes or are beyond our ability to comply. In this case, the risk manager will endeavour to negotiate changes in the language to bring them in line with the approved affiliation agreement. 1. An affiliation agreement is a formal contract between UW-Milwaukee and the employment agency (it is a non-UW-Milwaukee institution or a company in which the student/students have experience). It refers to the responsibilities and commitments of the various contracting parties that have been the subject of this agreement. Students who participate in these internships are not paid and experience is required to graduate. UW-Milwaukee does not require affiliation agreements. These contracts are signed by the agency and the director of university security and insurance, not by the dean of a college or department staff.
Many departments, schools and higher education institutions require or encourage students to take clinical or field training as part of the degree program required. This training experience is usually obtained in collaboration with an external agency that agrees to participate in cooperation with the school or university concerned. It is in the interest of both parties that a formal agreement be reached before including students in such programs. From a risk management perspective, it is particularly advantageous for the university department to negotiate a contractual agreement that clearly delineates the level of stakeholder responsibility in line with our risk management objectives. A. Master Agreement These documents contain the main conditions approved by the University of Wisconsin Council of Regents on behalf of the university and affiliate agency. A program memorandum is a document describing student placement.
It defines the content of the course, the expectations of the student and the Agency and may include specific requirements.
